I am here to help you develop a care plan so you can enjoy your loved one’s senior years with them, stress-free.
Together, we’ll develop and implement a care plan that accounts for your specific needs and budget. Which ultimately means helping your loved ones stay healthier, happier, and at home longer.
To begin the process, we will schedule three half-hour telehealth assessments to go over your parents’ health, current care plans, goals, needs, and budget.
Care Plan Assessment
Inital assessment to determine care plan- One monthly 60-minute session
- Two 45-minute check-in telephone call
- Oversight by a Licensed Master Social Worker
Once the initial assessment has been complete, I will sit down and develop your customized care plan taking into account your priorities, health, and family goals. Based on your plan I would recommend one of the following care plans.
“Two Eyes Are Better Than One”
When you just need someone to check in on your loved one and be there in case of an emergency.- One monthly 60-minute session
- Two 45-minute check-in telephone call
- Oversight by a Licensed Master Social Worker
“Road Maps”
Laying out the initial roadmap for care and providing some guidance and support as needed.- Initial phone consultation
- Home visit: Well-Being & Safety Evaluation
- Four monthly 60-minute session
- Plan and identify immediate concerns
- Support plan for primary caregiver (s)
- Review of resources for local entitlements, alternative care options
“Smooth Aging Care”
A month and a half of care- Initial phone consultation
- Home visit: Well-Being & Safety Evaluation
- Six monthly 60-minute session
- Plan and identify immediate concerns
- Support plan for primary caregiver(s)
- Review of resources for local entitlements, alternative care options
- 30 days of on-going professional support to monitor and manage plan of care
- Oversight by a Licensed Master Social Worker

In-depth care, support, and guidance. I am your partner in care.- Initial phone consultation
- Home visit: Well-Being & Safety Evaluation
- 12 monthly 60-minute session
- Develop a plan of care for immediate concerns
- Develop a plan of support for primary caregiver(s)
- Review of resources for local entitlements, alternative care options
- 60 days of on-going professional support to monitor and manage plan of care
- Oversight by a Licensed Master Social Worker
